Acute psychosis more common in older mums: study

First-time mothers who give birth after the age of 35 are more than twice as likely as younger mums to suffer postpartum psychosis that could involve a danger to the newborn child, a Swedish study showed on Tuesday. “The risk of developing psychosis during the first 90 days (after childbirth) increased with age,” said the study, conducted by researchers at the Karolinska Institute in Stockholm and published by the Public Library of Science medical journal.

Women over 35 when they gave birth for the first time were 2.4 times more likely to develop postpartum psychosis than those younger than 19, according to the study, which was based on data gathered from all the nearly 750,000 first-time mothers who gave birth in Sweden between 1983 and 2000.

While some 80 percent of new mothers experience some kind of mental disturbance or light depression, only about one in 1,000 women suffer from actual psychosis in the first months after giving birth, the study showed.

Postpartum psychosis is defined as a serious mental disorder, involving delusions, hallucinations, severe eating or sleeping disturbances, suicidal tendencies and can even pose a threat to the newborn baby.

It demands immediate medical attention including the administration of anti-psychotic drugs and hospitalisation.

Most women who suffer from such a psychosis have prior psychotic histories, but the Karolinska research indicated nearly 50 percent of the cases appeared in women “without prior psychotic hospitalisation.”

The study meanwhile showed that factors including smoking and not living with the baby’s father had no impact on whether a woman developed postpartum psychosis, while suffering from diabetes and giving birth to babies with high birth weights reduced the risk.

Problems Tied to Obesity Also Seem to Affect Sleep

Three conditions often linked to obesity have also been tied independently to sleep apnea, new studies show.Insulin resistance, the progression of liver disease, and living a less-than-active life were all found to be associated with the common breathing disorder, regardless of people’s weight, according to reports published in the February issue of the American Journal of Respiratory and Critical Care Medicine.

A John Hopkins University study found a strong tie between insulin resistance — the body’s inability to metabolize glucose — and sleep-disordered breathing (SDB), pauses or other abnormalities in breathing while sleeping.

“What our research tells us is that SDB is characterized by multiple physiological deficits that increase the predisposition for type 2 diabetes mellitus,” study leader Dr. Naresh Punjabi, an associate professor of medicine and epidemiology at Johns Hopkins University School of Medicine, said in an American Thoracic Society news release.

Another Hopkins study found that obese people with chronic intermittent hypoxia, the lack of oxygen that occurs during obstructive sleep apnea (OSA), showed liver issues in proportion to the severity of the sleep disorder.

“We hypothesize that severe obesity, per se, acts as a first hit in the progression of liver disease, inducing hepatic steatosis, whereas the presence of the chronic intermittent hypoxemia that often characterizes OSA acts as a second hit,” lead researcher Dr. Vsevolod Y. Polotsky, of the Johns Hopkins Asthma and Allergy Center, said in the same news release. “The hypoxic stress of OSA may induce oxidative stress in the livers of patients with severe obesity, leading to further inflammation.”

Enzyme levels and other findings, though, suggest that obesity and sleep apnea are not completely tied to each other, he said, meaning that each condition must be dealt with separately to also address the complications of both.

“Our data suggest that patients with OSA and severe nocturnal hypoxemia should be screened for liver disease, and, conversely, patients with liver disease should be screened for OSA,” Polotsky said.

The third study found that excessive sitting or standing during the day causes a fluid shift in the legs during sleep that may have a role in the development of sleep apnea.

When people lie down to sleep, fluid that has been retained in the legs during the day gets redistributed to the upper body, Dr. T. Douglas Bradley, professor of medicine and director of the Centre for Sleep Medicine and Circadian Biology at the University of Toronto, explained in the news release. “It is, therefore, plausible that some of the displaced fluid might reach the neck and predispose one to upper airway constriction,” he said.

The researchers found these changes in people who were sedentary but not obese and who they suspected had obstructive sleep apnea — a discovery that might help explain why 40 percent of people with the breathing disorder are not obese and why exercise without weight loss appears to reduce sleep apnea issues in some people.

“An important implication of our observations is that sedentary living may predispose to OSA, not only by promoting obesity but also by causing dependent fluid accumulation in the legs, which can shift rostrally to the neck overnight,” Bradley said.

A global review into the effect of modern office design led to the conclusion that working in an open-plan had a negative impact with high levels of stress, conflict, high blood pressure, and a high staff turnover.

“The evidence we found was absolutely shocking,”” News.com.au quoted Dr Vinesh Oommen from the Queensland University of Technology’’s Institute of Health and Biomedical Innovation as saying.

“In 90 per cent of the research, the outcome of working in an open-plan office was seen as negative, with open-plan offices causing high levels of stress, conflict, high blood pressure, and a high staff turnover.

“The high level of noise causes employees to lose concentration, leading to low productivity, there are privacy issues because everyone can see what you are doing on the computer or hear what you are saying on the phone, and there is a feeling of insecurity,’’ he added.

There are increased odds of workplace conflict by “sitting so close to someone that each time their phone rings you can get irritated””.

“I think most of us, including myself, can relate to that,”” said Oommen.

Moreover, large, open spaces could lead to an increased risk of illnesses. For instance, influenza virus are passed on more swiftly.

“Based on these findings, I think employers around the country need to rethink the open-plan environment in their offices,”” he said.

Indian stem cell treatment gives hope to stroke patients

For the first time, Indian researchers claimed to have successfully used stem cells to treat brain damage resulting from stroke in mice, thus taking a step nearer to give hope to stroke patients suffering from permanent paralysis, disability and dependence on others.”We found that stem cells, developed by us, when injected in the damaged portion of the brain did trigger development of new brain cells without any life-threatening side-effects,” S Prabhakar, head of the department of neurology, PGI Chandigarh said.

“This is the first time that interactions between the two kinds of cells (injected cells and host cells) worked out,” he said.

Five centres across the country, including AIIMS, are conducting studies on the stem cell treatment of stroke, according to Prabhakar.

Brain stroke occurs when a blood vessel carrying oxygen and nutrients to the brain is blocked by a clot or bursts, causing the brain to starve.

“If deprived of oxygen for even a short period of time, the brain cells begin to die. Once this happens the part of the body controlled by that section of the brain is affected, causing paralysis,” M V Padma, a neurologist at AIIMS, said. Human trials for the study will begin this month. “From January onwards the trials in human being will start. In our experiment we wanted that once the stem cells were injected, it should reach and form new cells in the damaged destination and it really did.” Prabhakar said.There are two types of strokes. In the first, blood clots in vessels leading to blocking of the arteries in the brain. The second type occurs when a blood vessel ruptures causing a bleeding also known as hemorrhagic stroke.

There are also “mini-strokes” which is often ignored and which if left untreated will lead to full blown strokes.

Brain stroke is the third largest killer in India after heart attack and cancer and the second largest in the world, according to the World Health Organisation.

A WHO study, stated that the incidence of the disease in India to be around 130 per 100,000 people every year and says about 20 per cent of heart patients are susceptible to it.

“For this study, we injected human stem cells into adult mice only after deliberately inducing brain stroke in them,” Prabhakar said while explaining the experiment on the mice.

For this, the blood supply in the brain of mice was cut off for just 15 minutes and then restored. “That causes massive damage to the brain. It’s the sort of thing that happens when you have a cardiac arrest. Then human cells were injected and within about a week, the responses of mice improved dramatically,” he said.

Social Security overestimates death rates: study

The U.S. Social Security Administration, which pays out $600 billion a year in benefits to retirees, may have underestimated how a decline in smoking will increase life expectancy, two experts reported on Monday.Haidong Wang of the University of Washington in Seattle and Samuel Preston of the University of Pennsylvania said their calculations showed that by 2035 a man’s odds of surviving from age 50 to age 85 will be 22.5 percent greater than projected, and a woman’s odds more than 7 percent greater.

This could affect the Social Security Administration’s budget, which now calculates that by 2034, 74 million Americans over the age of 65 will be living, compared to 38.6 million now.

“What may be bad news for the fiscal balance of the Social Security system is good news for the population as a whole and especially for men,” they wrote in the Proceedings of the National Academy of Sciences.

“Because of changes in smoking behavior that have already occurred or that can be reliably projected, American mortality is likely to fall more rapidly than is commonly anticipated.”

In November, the U.S. Centers for Disease Control and Prevention reported that the U.S. adult smoking rate fell below 20 percent for the first time on record. It was 42 percent in 1964.

Smoking and secondhand smoke kill 443,000 people annually from cancer, lung disease, heart disease and other causes, the CDC says. Half of all long-term smokers, especially those who start as teens, die prematurely, many in middle age.

“Although epidemiologic studies have identified cigarette smoking as an important risk factor in mortality for at least a half century, there is no consensus on how great the risk is,” Preston and Wang wrote.

They noted that an American Cancer Society study showed that white male smokers aged 40 to 84 were 91 percent more likely to die than non-smokers and white women 46 percent more likely to die than non-smokers.

When they took into account the drop in smoking rates, Preston and Wang projected that mortality rates would fall more quickly by 2034 than the Social Security Administration has predicted.

The government projection gives a 50-year-old man only a 39 percent probability of living to age 85 in 2034, compared to Wang and Preston’s estimate of 57 percent.

Scientists find a gene that makes cancer spread

A single gene appears to play a crucial role in deadly breast cancers, increasing the chances the cancer will spread and making it resistant to chemotherapy, U.S. researchers said on Monday. They found people with aggressive breast cancers have abnormal genetic alterations in a gene called MTDH, and drugs that block the gene could keep local tumors from metastasizing or spreading, increasing a woman’s chances for survival.

“Not only has a new metastasis gene been identified, but this also is one of a few such genes for which the exact mode of action has been elucidated,” said Dr. Michael Reiss of The Cancer Institute of New Jersey in New Brunswick, whose study appears in the journal Cancer Cell.

“That gives us a real shot at developing a drug that will inhibit metastasis,” he said in a statement.

Stopping cancer’s spread is important — while more than 98 percent of patients with breast cancer that has not spread live five years or more, only 27 percent of patients whose cancer has spread to other organs survive.

Reiss and Yibin Kang of Princeton University used several different research approaches to find the gene, which helps tumor cells stick to blood vessels in distant organs.

To get them in the right general area, they used big computer databases of breast tumors and found that a small segment of human chromosome 8 was repeated many times in people with aggressive breast tumors.

While most normal DNA sequences contain only two copies of a gene, they found some breast tumors had as many as eight copies of this gene segment.

The team then turned to human breast tumor samples taken from 250 patients to look for these genetic abnormalities and found the gene MTDH was overly active or expressed in aggressive tumors.

EXISTS IN EVERY CELL

“This gene exists in every one of our cells,” Kang said in a telephone interview. “Somehow the tumor gains extra copies and overexpresses them.

“We saw 30 to 40 percent of them overexpressed this gene.”

The researchers then injected lab mice with tumor cells from patients who had this genetic alteration and found the mice formed tumors that were more likely to spread.

They also were more likely to resist treatment with traditional chemotherapy drugs, such as paclitaxel.

But when they genetically altered these tumors, inhibiting the MTDH gene, the tumor cells were less able to spread and were more vulnerable to chemotherapy.

Kang said he is hopeful the finding will lead to drugs that not only keep breast cancer from spreading, but also make it more responsive to treatment.

“If we have a drug to inhibit this type of gene, one stone hits two birds,” Kang said.

He said MTDH may also play a role in other types of cancers, including prostate cancer. “It’s likely to be a broad influence gene,” he said.

Kang said he thinks it would be possible to develop an antibody to neutralize the activity of the gene.

Already, it has gained the attention of drugmakers. Kang said he plans to meet with Johnson & Johnson next week.

“I’m quite optimistic we will try to develop a drug as quickly as possible,” he said.
